Register with us as a New Patient
How to register
We accept patients from Peacehaven and Telscombe Cliffs.
All new patients are asked to present identification upon registration. Please provide two forms of identification; one with your address (eg:- utility bill) and one photo ID.
We accept the following forms of Identification
- Birth certificate
- Marriage Certificate
- Medical card
- Driving Licence
- Passport
- Local Authority Rent card
- Paid Utility Bills
- Bank Cards / Statements
- National Insurance Number card
- Wage Slip
- Evidence of Benefit Entitlement (letter about benefits, exemption certificate etc.)
Registration
To apply to register as a patient here, you will need to complete a standard NHS registration form.
- You can do this by using the new NHS Register with a GP Service.
- You can also download, print and complete a paper copy of the registration form. We have printed copies for you at our reception desk if you can't print it yourself.
Complete the forms fully, otherwise there may be delays to your registration.
Please allow up to 2 days for the registration to be done.
We reserve the right to reject any registrations for patients who live outside of our catchment area.
If you are planning on being in our area temporarily (up to 3 months), and wish to register as a patient here, you may register as a 'temporary resident'. This means you can receive care here whilst keeping your existing GP registration active at your normal place of residence.
Speak to our reception team to ask for a temporary registration form.
Your Named GP
Here at the HavensHealth we have a nominated named GP to be accountable for each of our patients. The purpose of this nomination is to ensure that our patients have a named individual who they know is responsible for the overall care they receive at the surgery.
Having a named GP does not mean that you are obliged to see that GP only; you are welcome to continue to make appointments to see any of the GPs at the surgery.
If you have a preference as to which GP is named accountable to your care or if you would like to find out who your named accountable GP is, please contact the surgery and we will be happy to accommodate this request for you.
