Changes to registration during Covid-19 Pandemic
Due to the current COVID-19 pandemic, patients are asked to register without coming into the practice. To register you will need to complete our online registration form.
All patients should also book in a New Patient Review with a pharmacist
How to register
We accept patients from Peacehaven and Telscombe Cliffs.
All new patients are required to present identification upon registration. You will need to provide two forms of identification; one with your address (eg:- utility bill) and one photo ID.
We accept the following forms of Identification
- Birth certificate
- Marriage Certificate
- Medical card
- Driving Licence
- Local Authority Rent card
- Paid Utility Bills
- Bank Cards / Statements
- National Insurance Number card
- Wage Slip
- Evidence of Benefit Entitlement (letter about benefits, exemption certificate etc.)
Please remember to also send us your proof of ID when registering. You can send this to us via our email address: firstname.lastname@example.org
Your Named GP
Here at the HavensHealth we have a nominated named GP to be accountable for each of our patients. The purpose of this nomination is to ensure that our patients have a named individual who they know is responsible for the overall care they receive at the surgery.
Having a named GP does not mean that you are obliged to see that GP only; you are welcome to continue to make appointments to see any of the GPs at the surgery.
If you have a preference as to which GP is named accountable to your care or if you would like to find out who your named accountable GP is, please contact the surgery and we will be happy to accommodate this request for you.
